Personal Care Coordinator (PCC)
At Enrollment
 | Places “Welcome” call to new patient |

 | Initiates “Welcome” mailing that may include educational materials |
 | Transfers patient to Registered Nurse (RN) for initial assessment and training |
Follow-up
 | Schedules activities for follow-up |
 | Places proactive refill calls to patient 10 days prior to need date |

 | Administers adherence program per protocol |
Registered Nurse (RN)
At Enrollment
 | Performs initial assessment and completes baseline evaluation |
 | Contacts physician for missing information if necessary |
 | Counsels patient on infusion process and technique, if applicable |
 | Assists patient in arrangement of homecare or home teach if needed |
 | Reviews patient checklist |

Follow-up
 | Performs follow-up assessment when due, according to disease protocol |

Pharmacist (RPh)
At Enrollment
 | Evaluates initial referral and ensures clinical guidelines met |
 | Obtains patient weight and verifies appropriate dosing |

Follow-up
